چکیده

It has been demonstrated that catheter ablation may be a reliable technique for curing ventricular arrhythmias originating from the great cardiac vein (GCV), and there are some challenges and risks with this therapeutic method. Although some reports described successful catheter ablation of ventricular tachycardias originating from the GCV, premature ventricular contractions (PVCs) with left bundle branch block (LBBB) and right bundle branch block( RBBB) with a right inferior axis originating from a single focus in the GCV has rarely been reported.

Two Different Left Bundle Branch Block Patterns of Premature Ventricular Contraction Originating from Right Coronary Cusp

Premature ventricular contractions (PVCs) or ventricular tachycardia (VT), with QRS morphologies of left bundle branch block (LBBB) and inferior axis can be ablated successfully from the right ventricular outflow tract (RVOT). However, it is not uncommon that PVCs or VT assumed to originate from the RVOT actually arise from the aortic root because of the proximity of the RVOT to the aortic root...

Left ventricular outflow tract tachycardia with preferential conduction and multiple exits.

A 50-year-old man with idiopathic premature ventricular contractions (PVCs; PVC 1, Figure 1A) and ventricular tachycardias (VTs) was referred for catheter ablation. Activation and pace mapping were performed at multiple sites in the right and left ventricular outflow tracts (RVOT and LVOT) (Figure 1). Nonirrigated radiofrequency (RF) applications delivered in the left coronary cusp where pacing...

Measurements of conduction times with catheter electrodes during pacing from different ventricular sites.

Filtered bipolar catheter electodes, I mm apart, were used to pace and record from the high right atrium, right ventricular apex, right ventricular outflow tract, right ventricular inflow tract, middle cardiac vein, great cardiac vein, and endocardium of the left ventricular septal surface. Right ventricular apex to middle cardiac vein and the middle cardiac vein to right ventricular apex condu...

Mapping and ablation of epicardial idiopathic ventricular arrhythmias from within the coronary venous system.

BACKGROUND The prevalence of epicardial idiopathic ventricular arrhythmias that can be ablated from within the coronary venous system (CVS) has not been described. METHODS AND RESULTS In a consecutive group of 189 patients with idiopathic ventricular arrhythmias referred for ablation, the site of origin (SOO) of ventricular tachycardia and/or premature ventricular contractions was determined ...

Ventricular arrhythmias near the distal great cardiac vein: challenging arrhythmia for ablation.

BACKGROUND Catheter ablation for ventricular arrhythmia (VA) near the distal great cardiac vein (GCV) is often challenging, and data are limited. METHODS AND RESULTS Analysis was performed in 30 patients (19 men; age, 52.8±15.5 years) who underwent catheter ablation for focal VA (11 ventricular tachycardia and 19 premature contractions) with early activation in the GCV (36.7±8.0 ms pre-QRS). ...
